The Lead Homeless Outreach Case Manager is a field position whose primary responsibility is the successful engagement of chronically homeless individuals in the City of Buffalo, assisting them with basic needs followed by housing. Responsible for the supervision of the outreach team, along with completing monthly reports to Director and ensure program is in compliance with funding regulations. Other responsibilities include:
- Maintain regular contact with street and shelter clients
- Navigate clients through the Department of Social Services as well as other government bodies to secure entitlement benefits
- Perform regular outreach to soup kitchens and shelters as indicated on the department outreach schedule
- House at two clients per month
- Document all links to services and treatments in department database
- Enter all case notes and other assessments into BAS-NET
- Keep quality of BAS-NET data at 97% (HUD requires 95%)
- Chair Outreach Committee of the WNY Coalition for the Homeless and/or other designated community group
- Facilitate weekly case conferences with department
- Participate in night and/or early morning outreach to the homeless with the Outreach Committee
- Prepare any and all reports that are required in a timely fashion
- Take an active role in the Hope Center activities and provide support services to clients entering the Hope Center
- Participate on any fundraising for Matt Urban and the Hope Center
- Other duties as required
